What is Sculptra, and how is it different from a dermal filler?
Sculptra is an injectable collagen biostimulator made from poly-L-lactic acid, commonly abbreviated to PLLA. Rather than relying primarily on an injectable gel to create immediate volume, Sculptra stimulates a gradual tissue response that supports the production of new collagen and other components of the extracellular matrix.
PLLA is a synthetic, biocompatible and biodegradable material that has been used in medicine for many years, including in dissolvable surgical sutures. Following injection, the PLLA microparticles stimulate activity within the surrounding tissues. As the particles are gradually broken down and absorbed by the body, newly formed collagen contributes to longer-term improvements in facial support, volume and skin quality.
This is the main difference between Sculptra and a traditional hyaluronic-acid dermal filler. Hyaluronic-acid fillers provide immediate correction through the physical presence of a gel. Sculptra works more gradually, with visible improvement developing over a course of treatment as collagen production increases.
Any fullness seen immediately after a Sculptra treatment is largely caused by the water used to prepare the product and by temporary swelling. This usually settles within several days and should not be mistaken for the final result.

What can Sculptra treat?
Sculptra may be considered when facial ageing or weight loss has led to broader changes in collagen, volume and support. Rather than targeting one line in isolation, Dr Nick assesses how selected areas relate to the face as a whole and plans treatment around anatomy, suitability and realistic goals.

Flattened or hollow cheeks
Age-related changes in collagen, bone support and facial fat can make the cheeks appear flatter or more hollow. In suitable patients, Sculptra may gradually improve mid-face support and contour.

Hollowing of the temples
Age-related volume loss can make the temples appear hollow and the upper face more angular. Sculptra may be considered in carefully selected patients to gradually improve contour.

Jawline and lower-face changes
Changes in collagen, facial fat and deeper structural support can reduce jawline definition and contribute to early lower-face laxity. Sculptra may be considered to improve support in selected areas, with the aim of a gradual change in overall lower-face contour.

Generalised age-related volume loss
Sculptra may be considered when volume loss affects several connected facial areas rather than one isolated feature. Dr Nick plans treatment across selected areas with the aim of a gradual, balanced improvement.

Facial hollowing following weight loss
Significant or rapid weight loss, including weight loss while using GLP-1 medication, can make facial hollowing and loss of support more noticeable. Sculptra may be considered after Dr Nick has assessed facial anatomy, general health and whether weight and facial changes are sufficiently stable.
Poly-L-lactic acid and collagen stimulation
Sculptra works by introducing poly-L-lactic acid microparticles into an appropriate tissue plane. These microparticles stimulate a gradual tissue response that supports fibroblast activity and the production of new collagen and other components of the extracellular matrix.
Initial treatment effect
Sculptra is supplied as a powder and reconstituted with sterile water before treatment. The injected fluid can create temporary fullness immediately after the procedure, but this usually subsides over the following few days.
This early fullness is not the final result. The treated areas may temporarily return close to their original appearance before collagen-related improvement becomes visible.

Gradual collagen stimulation

The PLLA particles are gradually broken down and absorbed by the body. Newly formed collagen contributes to a progressive improvement in facial support, volume, firmness and overall skin quality.

A course of treatment
Sculptra is usually planned as a course rather than a one-off injection. Most patients require approximately two to three sessions, commonly spaced at least four to six weeks apart.
The precise number of sessions depends on your age, anatomy, skin quality, degree of volume loss and treatment goals. A conservative amount is used initially because the collagen response continues to develop after each session.

Product preparation
Sculptra is supplied as a powder and reconstituted with sterile water before injection. Lidocaine may be added where appropriate to improve comfort during treatment.
The treatment area is carefully cleansed and marked before the injections begin.
Precise injection
Sculptra is placed into the appropriate deep dermal, subcutaneous or supraperiosteal plane, depending on the anatomy and intended treatment outcome. A needle or cannula may be used according to the area being treated.
The injection session commonly takes approximately 30–45 minutes, although the full appointment will be longer to allow for assessment, preparation and aftercare.
Aftercare and massage
Immediately after treatment, Dr Nick will massage the treated areas to help distribute the product evenly. You will then be instructed to massage the areas for five minutes, five times a day, for five days — the established “5-5-5” Sculptra aftercare routine.
You will receive clear instructions on how to massage the areas safely and what to avoid during the early healing period.
Gradual results and review
Any initial fullness from the injected fluid usually settles within several days. The true Sculptra result then develops progressively as collagen production increases.
Your response will be reviewed before further treatment. Additional sessions should not be performed sooner than four weeks after the previous treatment, allowing time to assess the developing correction and avoid overtreatment.

After your Sculptra treatment
Swelling, redness, tenderness, bruising or itching can occur after Sculptra treatment. The treated areas may initially appear fuller because of the injected fluid and temporary swelling. This usually subsides over the following few days.
You will generally be advised to:
- Massage the treated areas for five minutes, five times a day, for five days.
- Use clean hands and an appropriate cream when massaging.
- Avoid strenuous exercise for approximately 24–48 hours.
- Avoid saunas, steam rooms, sunbeds and excessive heat while swelling or redness is present.
- Avoid applying makeup until the initial injection points have closed.
- Avoid pressing or manipulating the areas outside the recommended massage routine.
- Follow the individual written aftercare provided by Dr Nick.
Contact the clinic if you develop persistent or increasing pain, marked asymmetry, unusual skin discolouration, visual symptoms, prolonged swelling or any lumps that do not improve as expected.

How many treatments will you need?

Radiesse

Sculptra cannot correct every cause of facial ageing. Significant skin laxity, heavy tissue descent or loss of skeletal support may require energy-based treatment, surgery or a combined approach.
When may Sculptra not be suitable?
Sculptra must not be used in patients with a known allergy to any of its components or a history of keloid formation or hypertrophic scarring.
Treatment should be deferred where there is active infection, inflammation or another active skin condition affecting or close to the proposed treatment area.
Sculptra may also be unsuitable or require additional assessment if you:
- Are pregnant or breastfeeding.
- Have a significant autoimmune, connective-tissue or inflammatory condition.
- Take medication that affects immune function, bleeding or healing.
- Have a bleeding disorder or take anticoagulant or antiplatelet medication.
- Have permanent filler, implants or an unidentified product in the treatment area.
- Have recently undergone another facial procedure.
- Have unrealistic expectations or want an immediate result.
Safety during pregnancy and breastfeeding has not been established. Dr Nick will review your full medical and treatment history before confirming whether Sculptra is appropriate.
Side effects and risks
Sculptra has an established safety profile when used in appropriately selected patients and administered by a suitably trained medical practitioner. However, all injectable treatments carry potential risks.
Common temporary effects include:
- Swelling
- Redness
- Bruising
- Tenderness or discomfort
- Itching
- Bleeding at the injection points
- Temporary asymmetry while swelling settles
Papules or nodules can occasionally develop beneath the skin. These may appear weeks or months after treatment and may be palpable without being visible. Less commonly, nodules can become visible, inflamed or require further medical management.
Appropriate product preparation, correct injection depth, conservative treatment and adherence to the post-treatment massage instructions can help reduce the risk, but they cannot eliminate it completely.
Other possible complications include infection, prolonged swelling, delayed inflammatory reactions, skin discolouration and an unsatisfactory cosmetic result. Rarely, injectable treatments can be associated with vascular complications.
Unlike hyaluronic-acid fillers, Sculptra cannot be dissolved with hyaluronidase, so careful patient selection, preparation and precise injection technique are particularly important.
