That’s why filler alone doesn’t always fully correct them.

Understanding marionette lines

What are marionette lines?

Marionette lines are the paired creases that run from the corners of the mouth obliquely down toward the jaw and chin, named for their resemblance to the hinged mouth of a puppet. The line itself forms where a fixed ligament in the jaw tissue tethers the skin in place, while the fat sitting either side of it (particularly beneath the lower lip) loses volume and thins unevenly with age. As with nasolabial folds, this is a structural, volume-related change, not simply a surface wrinkle.

The cause

What causes marionette lines?

Marionette lines have a genuinely dual cause, and understanding both halves matters for treatment:

  • Structural: ligament tethering and volume loss. A fixed ligament anchors the skin at the jawline, while the fat compartment beneath the lower lip and along the jaw loses volume and descends with age and bone remodelling. The fixed tether plus the deflating tissue either side is what creates the visible line.
  • Muscular: the depressor anguli oris. This small muscle pulls the corners of the mouth downward every time it contracts, which happens with certain expressions and, for some people, more habitually than others. Its ongoing pull actively worsens the downturned appearance on top of the structural change above.

Because both a structural and a muscular component are usually at play, a downturned mouth that is strongly muscle-driven may not fully respond to volume replacement alone: a point worth understanding before treatment, and covered further below.

The two safety points that matter here

1. This is a recognised vascular danger zone for filler. An artery supplying the lower lip runs directly beneath the muscle that pulls your mouth corners down, and the facial artery courses through this same injection field. A documented case report describes vascular occlusion following low-volume chin filler in this exact region, with warning signs including pain, colour change and a mottled, bluish discolouration of the skin. This is why marionette line and chin filler should only ever be performed by an appropriately trained medical injector using technique (blunt cannulas, small volumes, slow injection) that reduces this risk, with hyaluronidase available if needed.

2. Sudden, one-sided facial drooping is a medical emergency, not a cosmetic concern, and this is categorically different from ordinary marionette lines. Ordinary marionette lines develop gradually, over years, and are broadly symmetrical. A mouth corner (or eyelid) that droops suddenly on one side only, developing over hours or days, can be a sign of Bell’s palsy or, together with other signs such as arm weakness or slurred speech, a stroke: a medical emergency requiring an immediate 999 call or A&E attendance, not a cosmetic booking.

Overfilling is a further documented risk. Aggressively treating this area, or repeated treatment over time, can itself produce downward-turned mouth corners as part of a recognised pattern of overfilled, heavy-looking tissue: the opposite of the intended result. First-line correction for this is targeted hyaluronidase, and it is far easier to avoid with conservative, well-planned treatment from the outset.

NHS vs private

Are marionette lines treated on the NHS?

No. Marionette line treatment (whether with dermal filler or anti-wrinkle injections) is an elective cosmetic treatment, and NHS commissioning policy is explicit that non-surgical treatments such as line filling for facial ageing are not funded.
