A double chin is usually ordinary submental fat: soft, symmetrical, and unrelated to weight loss efforts alone. But a lump is not a chin: any hard, fixed or growing lump in the neck or under the jaw needs GP assessment before anyone considers cosmetic treatment.
What is a double chin?
“Double chin” is the everyday term for submental fat: a localised pocket of fat below the chin and along the jawline, sitting just under the skin. It is assessed clinically using standardised rating scales, and the treatment-trial literature consistently frames it as a genuine, common cosmetic concern independent of a person’s overall weight.
Not every case is the same. Assessment needs to distinguish fat-predominant fullness (where fat reduction treatments work well) from cases where skin laxity or fat sitting beneath the muscle layer is the main driver. In the research trials this distinction actually excluded some patients from fat-reduction treatment altogether, because it doesn’t give a good result when skin laxity, rather than fat volume, is doing most of the work.
What causes a double chin?
Genetics plays a substantial role: some people carry submental fat regardless of their overall body weight, which is why diet and exercise alone often don’t shift it. Ageing-related skin laxity is a separate, frequently overlapping factor: as skin loses elasticity, the appearance of fullness under the chin can look more pronounced even without a true increase in fat.
Because skin laxity, subplatysmal (under-muscle) fat, and simple subcutaneous fat can all look similar at a glance but respond very differently to treatment, working out which one you actually have is the first real step, not guesswork.
A lump is not a chin
Ordinary submental fat is soft, roughly symmetrical, and compresses like normal fatty tissue; it is not a discrete hard mass. A GP assessment, not cosmetic treatment, is the right first step if your lump is hard and does not move, or lasts more than two weeks, or you notice a swelling on the side of your neck that does not go down within two weeks.
Swollen lymph nodes. A submental or neck lymph node that is enlarging, hasn’t gone down within a week, feels hard or doesn’t move under your fingers, or comes with night sweats or a high temperature, should be checked by a GP. Difficulty swallowing or breathing alongside swollen glands needs same-day medical help.
Goitre (thyroid swelling). A goitre is a lump or swelling at the front of the neck caused by a swollen thyroid, and usually appears at the lower front of the neck, lower and more central than the fat pad under the chin. A hoarse voice, a feeling of throat obstruction, wheezing or difficulty swallowing point toward a thyroid cause rather than fat, and should be seen by a GP.
Salivary gland swelling. The submandibular glands sit deeper and further back, beneath and in front of the angle of the jaw, with a firmer, rubbery feel on examination. The most common cause is a salivary stone, which classically causes swelling that comes on with eating: a pattern completely unlike static submental fat. Any salivary swelling needs referral and investigation, especially if it enlarges quickly or feels fixed to the skin.
When it’s genuinely urgent. UK cancer-referral guidance treats an unexplained lump in the neck, in people aged 45 and over, as a reason to consider an urgent two-week specialist referral, and an unexplained thyroid lump alone is enough to trigger the same pathway, at any age. This isn’t meant to alarm anyone with ordinary submental fullness; it’s why the distinction matters, and why assessment always comes first here.
Is double chin treatment available on the NHS?
No. cosmetic submental fat treatment is not NHS-funded. The NHS position is direct: “cosmetic surgery is not routinely provided on the NHS,” and double-chin treatment does not appear among the narrow list of medical exceptions.
Where the safety checks above identify a genuine lump, lymphadenopathy, goitre or salivary gland problem rather than ordinary fat, that is a completely different, NHS-funded pathway: GP assessment, routine referral, or the urgent two-week suspected-cancer route where the criteria are met. This isn’t a cosmetic decision at all; it’s triggered by clinical suspicion, not preference.
So the honest summary is this. If you have a lump rather than diffuse fullness, or any of the red flags above, start with your GP. If what you have is straightforward, soft, symmetrical submental fat, that is a private, cosmetic consultation.
