“Acne scar removal” is a common search term, but it isn’t fully accurate. Atrophic acne scarring can be visibly and measurably improved with the right treatment matched to the right scar type — it cannot be made to disappear completely. What is acne scarring, and why can’t it be fully removed?

Most acne scarring is atrophic — a loss of collagen and tissue left behind where an inflamed spot damaged the deeper skin as it healed. That damaged collagen structure does not simply regenerate back to its original state, which is why no laser, peel or needling device can promise complete removal. What every evidence-based treatment can do is stimulate new collagen, resurface the surface texture and soften the visible depth of the scar — a genuine, often significant improvement, but a management process rather than a cure.

There are three common shapes of atrophic scar — ice-pick (narrow and deep), boxcar (wider with sharply defined edges) and rolling (broad with a sloping, wave-like edge) — and they don’t all respond to the same treatment in the same way. There is no single “best” acne scar treatment — the right choice depends on the scar. Here is an honest guide to how our options compare, so you understand why Dr Nick may recommend one, or a combination, for you.

Ice-pick & deep boxcar scars

Deep, narrow ice-pick scars often respond poorly to resurfacing alone. TCA CROSS or punch techniques may be more appropriate for selected scars; these are not currently provided at Rejuvena, so Dr Nick will explain when specialist referral is the more honest option. Broader boxcar texture may suit cautious laser or microneedling after assessment.

Rolling & tethered scars

Rolling scars can combine surface texture with fibrous bands beneath the skin. Laser or RF microneedling may improve texture and collagen, but strong tethering may need subcision, which Rejuvena does not currently offer. We will refer rather than imply that a device can release every tethered scar.

Shallow, widespread scarring

Milder, more generalised texture change often responds well to a course of SkinPen microneedling, with BioRePeel and Dermalux LED supporting skin quality between sessions — a gentler route with less downtime.

Deep or mixed scarring frequently needs a combination approach rather than one device alone. Your exact plan — which treatments, in what order, how many sessions — is agreed with Dr Nick at your consultation, not decided in advance of seeing your skin.

Candidacy, safety & honest limitations

Acne scar treatment suits adults with atrophic (indented) acne scarring — ice-pick, boxcar, rolling or a mixed pattern — whose active acne is settled or well controlled, and who want a realistic, doctor-assessed improvement in texture and depth rather than a guaranteed “fix.”

It is not suitable for everyone, and not always straight away. Treatment is usually deferred while acne is still actively inflamed, during pregnancy or breastfeeding for laser and RF options, and over active skin infection in the treatment area. Current or recent isotretinoin must be discussed with Dr Nick: timing depends on the proposed procedure, dose, healing history and advice from the prescriber rather than a blanket waiting period. A history of keloid or hypertrophic scarring needs careful, conservative assessment.

Honest limitations. No treatment can remove atrophic acne scarring completely. Deep ice-pick scars may need TCA CROSS or punch techniques, and strongly tethered rolling scars may need subcision; Rejuvena does not currently provide those procedures and will refer when they are more appropriate. BioRePeel and Dermalux can support post-acne marks, skin quality or recovery, but they do not rebuild deep structural scars. Darker skin tones also carry a higher risk of post-inflammatory pigmentation after aggressive resurfacing, so settings are conservative and a test area may be advised.

Can acne scars be completely removed?
No — and we would rather tell you that honestly than sell you a “removal” that doesn’t exist. Atrophic acne scars are areas of lost collagen; treatment stimulates new collagen and resurfaces the skin to significantly soften depth and texture, but the scar tissue cannot be perfectly rebuilt to match the surrounding skin. Improvement, not erasure, is the realistic and achievable goal.
Which treatment is best for my type of acne scar?

Redness, swelling, crusting or peeling can create several days of visible social downtime; the depth of treatment, area and individual healing determine when normal activities and skincare can safely resume.
Are these treatments safe for darker skin tones?
They can be, but darker skin tones carry a higher risk of post-inflammatory pigmentation with laser and more aggressive resurfacing, so settings are chosen conservatively and a test patch may be advised. Dr Nick assesses your Fitzpatrick skin type and history at consultation and will recommend the safest suitable option — sometimes a gentler treatment such as SkinPen or BioRePeel is preferred over more aggressive laser settings.
Can I treat acne scars while I still have active acne?
Generally no — active, inflamed acne is addressed and brought under control first. Treating scar revision over active breakouts risks creating new scarring during the process. Dr Nick will advise on managing active acne before starting a scar-revision plan.
Does the NHS treat acne scars?
Sometimes, particularly for severe scarring. NICE guidance says severe acne scarring that persists for a year after acne has cleared should be considered for referral to a consultant dermatologist-led team. Access and funding vary locally, so speak to your GP; mild cosmetic scarring is often managed privately.
What is the difference between ice-pick, boxcar and rolling scars?
Ice-pick scars are narrow and deep, like a small puncture. Boxcar scars are wider with sharply defined, box-like edges. Rolling scars are broad with a sloping, wave-like edge caused by fibrous bands tethering the skin beneath. They respond differently to treatment, which is why an accurate assessment matters more than the treatment name itself.
