Understanding snoring

Snoring is the sound produced when air passes through a narrowed or partially obstructed upper airway and causes nearby soft tissue to vibrate.

What is Romeo™ anti-snoring laser treatment?

Romeo™ is a fractional Er:YAG laser treatment designed to reduce snoring in appropriately selected adults. It is performed using the Romeo™ handpiece with the Asclepion Dermablate laser at a wavelength of 2,940 nm.

During treatment, controlled fractional laser pulses are delivered to selected areas of the soft palate, uvula and surrounding accessible tissue. These pulses create small, precisely distributed treatment points within the oral mucosa.

The immediate thermal response causes contraction within the treated tissue. The subsequent healing response is intended to support collagen remodelling, making the tissue firmer and less prone to vibration during sleep.

Romeo™ does not remove tissue and does not involve surgical incisions. However, because it produces a controlled fractional response in the oral mucosa, it is more accurately described as a minimally invasive, non-surgical treatment rather than completely non-invasive.

The aim is to reduce the intensity or frequency of snoring — not to guarantee its complete elimination. Results depend on whether soft-palate vibration is an important cause of that person’s snoring.

Why does snoring happen?

The cause is not the same for everyone. Contributing factors can include the soft palate and uvula, the tongue position, nasal obstruction, enlarged tonsils, excess weight, alcohol, sedative medication, smoking, sleeping position and individual jaw or airway anatomy.

Romeo™ specifically targets accessible soft tissue inside the mouth. It will not correct every possible cause of snoring, which is why careful assessment is essential.

Important distinction

Snoring and obstructive sleep apnoea are not the same

Simple or primary snoring is noise produced by vibrating tissue during sleep without repeated episodes of clinically significant breathing interruption.

Obstructive sleep apnoea, or OSA, is a medical condition in which the upper airway repeatedly narrows or closes during sleep, interrupting breathing and potentially reducing blood-oxygen levels.

Possible warning signs include:

  • Gasping, choking or snorting during sleep
  • Loud, irregular snoring
  • Repeated waking during the night
  • Morning headaches
  • Persistent daytime sleepiness
  • Reduced concentration
  • High blood pressure

Romeo™ is offered for selected snoring presentations. It is not a treatment for obstructive sleep apnoea and must not replace CPAP, a mandibular-advancement device or another treatment recommended for diagnosed OSA. If your assessment suggests an increased risk of OSA, treatment will be deferred and you will be advised to consult your GP or an appropriate sleep service. A sleep study may be required.

Medical consultation

Your consultation includes a history of your snoring, relevant medical conditions and medication, previous nasal or throat treatment, weight and lifestyle factors, daytime symptoms and observations made by your partner.

We will also consider whether the pattern raises concern about obstructive sleep apnoea. Screening may include recognised tools such as the Epworth Sleepiness Scale, but questionnaires do not replace a sleep study when OSA is suspected.

Mouth and airway assessment

Dr Nick examines the mouth and accessible upper-airway anatomy, including the soft palate, uvula, tonsillar region, tongue position and ease of treatment access.

Romeo™ is most relevant where vibration of accessible soft tissue appears to be an important contributor. Nasal obstruction, enlarged tonsils, significant tongue-base obstruction or other anatomical causes may require assessment elsewhere.

Preparing for treatment

You should avoid alcohol on the day before and the day of treatment, following the instructions provided by the clinic. Laser-protective eyewear is worn during the procedure.

Local anaesthetic is not usually required, although individual comfort and gag-reflex sensitivity are considered.

Laser treatment

You will sit in a slightly reclined position with your mouth open. The tongue is gently lowered so the Romeo™ handpiece can access the treatment area.

Controlled fractional pulses are applied in a planned pattern. Short pauses can be provided throughout the procedure. The laser-delivery stage is brief, but you should allow approximately 20–30 minutes for the complete appointment.

Immediately afterwards

The oral tissues may appear red, and very small treatment points or pinpoint bleeding may be visible. These are expected consequences of the fractional treatment rather than accidental injury.

You will receive written instructions concerning drinking, eating, smoking and alcohol.

Your treatment course

The manufacturer recommends between three to six treatments depending on the patient. In clinical studies, three sessions were commonly performed.

At Rejuvena, a course of up to six sessions approximately one month apart may be recommended. Your response is reviewed before further treatment is undertaken.

Snoring & you

Why persistent snoring deserves proper assessment

Snoring can disturb the sleep of both the person snoring and the person sleeping beside them. It may contribute to relationship strain, separate sleeping arrangements and concern about the quality of breathing during sleep.

However, snoring is a symptom rather than a single diagnosis. Its possible causes include:

  • Soft-palate or uvula vibration
  • Nasal blockage or septal deviation
  • Enlarged tonsils
  • Tongue position
  • Jaw and craniofacial anatomy
  • Excess weight or increased neck circumference
  • Alcohol or sedative medication
  • Smoking
  • Sleeping on the back
  • Obstructive sleep apnoea

A laser treatment aimed at the soft palate cannot correct every one of these causes. Appropriate patient selection is therefore one of the most important factors affecting the likelihood of improvement.

What results can Romeo™ produce?

The purpose of treatment is to reduce the intensity, frequency or disruptive effect of snoring. As the treated area is inside the mouth, results are assessed functionally rather than through conventional before-and-after photographs.

Changes may be monitored using:

  • Your own assessment of snoring
  • Reports from your sleeping partner
  • A snoring diary
  • Consistent audio recordings or an app used under similar conditions
  • Changes in sleep disruption associated with snoring

Some patients notice a change after an early session, while others require the full course. Some experience little or no meaningful improvement.

Clinical studies of Er:YAG treatment have reported reductions in patient- and partner-rated snoring, generally after two or three sessions. However, many studies are small, rely partly on subjective reporting and do not include untreated control groups.

Results are therefore variable and cannot be guaranteed.

What does the evidence say about Er:YAG laser treatment for snoring?

Research into Er:YAG laser treatment suggests that selected patients may experience a reduction in subjective snoring severity with generally good treatment tolerance.

A systematic review and meta-analysis covering seven studies and 247 participants reported an improvement in pooled snoring scores and an overall satisfaction rate of approximately 80%. It did not find a significant change in the apnoea–hypopnoea index or respiratory-disturbance index.

A prospective study of 40 patients reported reduced partner-rated snoring and generally mild treatment discomfort. However, the study did not include an untreated control group, and not every participant underwent repeat objective sleep testing.

A later sham-controlled trial also examined non-ablative 2,940 nm Er:YAG treatment, but the overall evidence base remains relatively small.

Taken together, the evidence suggests that Er:YAG treatment may help reduce snoring in carefully selected patients, but it should not be presented as a proven treatment for obstructive sleep apnoea or as guaranteed to improve wider health outcomes.

Aftercare

For the first two hours after treatment, drink only plain water at room temperature unless you are advised otherwise.

For approximately 24 hours, you should avoid:

  • Alcohol
  • Smoking or vaping
  • Carbonated drinks
  • Very hot or very cold drinks
  • Ice cream
  • Hard, sharp, dry or heavily grilled food
  • Food likely to scrape or irritate the treated area

Choose soft, mild foods and remain well hydrated. Follow the personalised instructions provided after your treatment.

Temporary redness, dryness, swelling, stinging, a burning or scraping sensation and discomfort when swallowing can occur. These effects generally settle within a few days, with many resolving within approximately 48 hours.

Contact the clinic promptly if swelling is increasing, you cannot swallow normally, pain is severe or worsening, you develop breathing difficulty, or you are otherwise concerned. Any breathing difficulty should be treated as an emergency.

Is Romeo™ the same as NightLase?

Does Romeo™ cure snoring?
No treatment can guarantee complete or permanent cessation of snoring. Romeo™ aims to reduce the intensity or frequency of suitable soft-palate-related snoring. Some patients respond well, while others experience limited or no meaningful improvement.
Can Romeo™ treat obstructive sleep apnoea?
No. Rejuvena does not offer Romeo™ as a treatment for obstructive sleep apnoea. If OSA is suspected, treatment is deferred and medical investigation is recommended.
How do I know whether I may have sleep apnoea?
Warning signs include witnessed breathing pauses, choking or gasping during sleep, loud irregular snoring, repeated waking, morning headaches and significant daytime sleepiness. Speak to your GP if you experience these symptoms. The NHS advises assessment because untreated sleep apnoea can be serious.
Will I need a sleep study?

Is the treatment painful?

How long does treatment take?
The laser-delivery stage is relatively brief. Allow approximately 20–30 minutes for preparation, treatment and short breaks during the complete appointment.
How many sessions will I need?

When might I notice a change?
Some people report an early reduction in snoring, but tissue healing and remodelling continue over the following weeks. The overall response is usually assessed after the recommended course.
Is there downtime?
Most patients do not need time away from work or routine daily activities. However, temporary throat discomfort can occur, and there are restrictions on food, drinks, alcohol and smoking after treatment.
What can I eat and drink afterwards?
Initially, room-temperature water is advised. For approximately 24 hours, avoid alcohol, smoking, carbonated drinks, extremes of temperature and hard or irritating foods. Follow the written instructions provided by the clinic.
How long do results last?
Duration varies. Some published studies describe maintained subjective improvement beyond one year in a proportion of patients, while others report a partial or complete reduction in effect. Maintenance should therefore be based on your individual response rather than promised annually.
What happens if it does not work?
If there is no meaningful improvement, further laser sessions may not be appropriate. Other contributing causes of snoring may need to be considered through your GP, dentist, ENT service or sleep specialist.

Is it right for you?

Romeo™ may be considered for an adult who experiences regular, disruptive snoring; has no diagnosed untreated obstructive sleep apnoea; has no concerning OSA symptoms requiring investigation; appears to have snoring associated with vibration of the soft palate, uvula or accessible palatal tissue; has anatomy that allows the treatment area to be accessed safely; is medically suitable for fractional intraoral laser treatment; understands that improvement is variable and complete cessation of snoring cannot be guaranteed; is willing to follow the required aftercare; and has realistic expectations about treatment and maintenance. Suitability depends on the cause of the snoring — not simply how loud it is.

When it isn’t suitable

Treatment will not normally be offered where there is diagnosed obstructive sleep apnoea that has not been appropriately assessed and managed; suspected OSA requiring GP or sleep-service investigation; snoring caused predominantly by nasal obstruction, significant tonsillar enlargement, tongue-base or laryngeal obstruction, or another cause the laser cannot reach; anatomy that prevents safe access to the treatment area; active infection, ulceration or significant inflammation inside the mouth or throat; pregnancy; a relevant photosensitising medication; a medical condition or medication that may impair healing or increase procedural risk; previous surgery or treatment that has significantly altered the relevant anatomy; an inability to tolerate treatment because of a pronounced gag reflex; or unrealistic expectations or a request for guaranteed cessation of snoring.

Safety, side effects & what to expect

Romeo™ is performed without surgical incisions and usually without local anaesthetic. Most published studies describe low procedural pain and mainly temporary adverse effects. Expected short-term effects can include warmth, stinging or burning during treatment, pinpoint bleeding from fractional treatment points, redness inside the mouth or throat, dryness, a swollen or tight sensation, sore-throat-like discomfort, scraping or irritation, and mild pain or discomfort when swallowing. These effects commonly settle over the following hours or days, although individual recovery varies.
