Before anything else, Dr Nick asks about your scar’s location, age, whether it crosses a joint, and whether it has ever been assessed by a burns service. Anything suggesting contracture, restricted movement, or new ulceration in a long-stable scar is referred, not treated cosmetically.
Skin assessment & honest expectations
If your scar is suitable, Dr Nick examines its depth, colour and texture, checks your Fitzpatrick skin type, and explains realistically what treatment can and can’t achieve for your specific scar before any plan is agreed.
Patch test for darker skin tones
Because darker skin carries a higher risk of post-inflammatory pigment change, a test patch and conservative first-session settings are often advised before treating a larger area.
Comfort measures
A topical anaesthetic is usually applied before laser or microneedling sessions. Most people describe the sensation as warm tingling or mild scratching rather than pain.

Aftercare & sun protection
Expect redness for a few days, sometimes light peeling. You’ll be advised on gentle cleansing, moisturising and strict broad-spectrum SPF — sun exposure on a healing scar increases the risk of it darkening.
Course & review
Burn scar improvement builds gradually over a course of sessions, usually spaced several weeks apart. Progress is reviewed at each visit, and the plan is adjusted honestly if a scar isn’t responding as hoped.
When we’d refer you, not treat you
Two things always come before any cosmetic treatment of a burn scar. First, contracture — a scar that has shrunk and tightened, particularly over a joint, restricting movement. That is a functional problem needing surgical reconstruction from a burns or plastics specialist, not a laser session at an aesthetic clinic, and the professional body for UK plastic surgeons has publicly warned patients to be wary of cosmetic-clinic advertising for exactly this kind of post-burn problem.
Second, malignant change. Rarely, a long-stable burn scar can develop a squamous cell carcinoma within it (Marjolin’s ulcer) — typically decades after the original injury. If a scar that has been quiet for years suddenly starts ulcerating, weeping, bleeding or growing, that needs prompt GP assessment, not cosmetic treatment. Neither of these is common. Most people asking about burn scar removal have an old, flat or mildly raised scar that isn’t restricting anything — and that’s exactly the scar we can help soften and refine here.
What burn scar removal can — and can’t — do
Who it suits
Adults with a settled, non-restrictive burn scar — flat or mildly raised, not tightening a joint, and confirmed stable at assessment — who want to improve texture, colour and pliability and are willing to commit to a course of sessions plus daily sun protection.
Who is not suitable — yet
Scars restricting joint movement (contracture), any scar that is newly ulcerating, bleeding or changing after years of stability, active infection or open skin in the area, pregnancy, and — for laser specifically — an active tan or recent sunbed use. These need a specialist opinion or time before cosmetic treatment.
The honest evidence
Systematic-review evidence suggests fractional laser approaches can improve selected burn-scar outcomes, but studies use different devices, settings, scar ages and outcome measures. That evidence does not establish one universal protocol or guarantee the same result with Er:YAG treatment. Your plan is therefore individualised and reviewed rather than sold as a fixed course.
With burn scars, the referral question comes before the treatment question, every time. Most scars we see can genuinely be softened. A minority need a burns or plastics specialist first, and we say so plainly rather than treating everything that walks through the door.
