Tear Troughs: Groove, Hollow or Wrinkle?
Why the tear trough groove can appear from your 20s while broader hollowing comes later, why under-eye wrinkles are a separate skin problem entirely, and doctor-led treatment in Bedford.
“Tear trough” gets used loosely, but it actually covers three genuinely different things: a fine structural groove some people have from their 20s, a broader age-related hollow from midface volume loss, and separate fine wrinkles caused by thin skin and muscle movement. Getting the right one matters, because they don’t all need the same treatment.
Groove, hollowness or wrinkle: which do you have?
The groove is the classic tear trough: a fine indentation sitting just below the orbital rim at the junction of the eyelid and cheek, anchored by a fixed ligament and muscle attachment. Because it is created by fixed anatomy rather than accumulated ageing, it can genuinely be visible from a young age: some people notice it in their 20s. This is structural, not a sign that you’re ageing badly.
Hollowness is a different, broader thing: a wider depression across the whole lower-eye and upper-cheek area, caused by age-related loss of the cheek’s deeper fat pad. This is the pattern that develops later, typically alongside other midface ageing changes, and can occur with or without the groove above.
Under-eye wrinkles are separate again: fine lines caused by the thinnest skin on the body (around 0.2mm in places) combined with repeated muscle movement from smiling and squinting, plus sun exposure. This is a skin-texture problem, not a volume problem, and it can appear in people who have no hollowing at all.

What causes each type?
- The groove: fixed ligament and muscle anatomy at the lid-cheek junction. Largely constitutional rather than primarily an age-related change.
- Hollowness: age-related atrophy of the deeper cheek fat pad, which becomes visible as it thins and the overlying tissue has less support.
- Wrinkles: repeated contraction of the muscle that circles the eye (the same mechanism behind crow’s feet), combined with UV exposure, which affects thin eyelid skin especially readily.
Because these mechanisms are genuinely different, the treatment that helps one does not necessarily help another: filling a groove that’s actually fine, textural wrinkling will not soften the lines, and treating wrinkles alone will not correct genuine volume loss.
The safety point for tear trough filler
Tear trough filler carries a real, documented risk of vascular occlusion and, rarely, permanent vision loss: the same vascular safety point covered in full on our eye bags page, which we don’t repeat here. In short: this area sits close to blood vessels connected directly to the arteries supplying the eye, and filler should only ever be performed by an appropriately trained injector using technique that reduces this risk.
What is genuinely new to flag here is that under-eye wrinkles will not respond to that same filler treatment: a skin-texture problem needs a skin-directed approach, not more volume. Treating fine lines with filler, when what’s actually needed is a skin-quality treatment, is a common mismatch worth avoiding.
Are tear troughs treated on the NHS?
No. Tear trough groove, hollowing and under-eye wrinkles are all elective cosmetic concerns, and cosmetic surgery and cosmetic procedures generally are not routinely provided on the NHS. There is no NHS pathway for any of the three presentations described above, because none is a medical condition requiring treatment.
