Understanding dark circles

Dark circles aren’t one condition — they’re vascular, structural or pigment, and often a mix. Get the cause wrong and the treatment won’t work.

The three causes of dark circles: vascular (bluish-purple tone from superficial veins), structural (shadow from a tear-trough hollow) and pigmentation (brownish tone from excess melanin).

The three causes of dark circles

Dark circles are caused by one or more of three underlying mechanisms — and most patients have a combination:

1. Vascular (bluish-purple tone)

Periorbital skin is the thinnest on the body (~0.5mm). In a vascular presentation, the superficial venous network shows through as a bluish or purple shadow — worsened by fatigue, allergies, genetics and ageing, and tends to fluctuate with tiredness.

2. Structural / volume loss (shadow from a hollow)

With age, periorbital fat pads and orbital bone volume diminish, creating the tear-trough groove. This concavity casts a shadow that reads as darkness regardless of skin colour — the dominant cause in patients over 35.

3. Pigmentation (brownish tone)

Some patients, particularly with medium to darker skin tones, have excess melanin in the periorbital skin itself — a brownish discolouration with a genetic component, often worsened by sun, eye-rubbing and allergy.

When it isn’t suitable

It may not be suitable during pregnancy or breastfeeding, or if you have an active infection, inflammation or skin condition around the eyes. IPL is not advised over recently tanned skin, and filler is avoided if you have a relevant allergy, a bleeding disorder or are taking certain blood thinners. Dr Nick will review your medical history and skin type at consultation and advise if treatment should be deferred or adapted.
